Peoria and Phoenix are no longer proctoring assessments at their facilities. Qualifications are still required and are still offered through CSA. CSA assessments are now delivered through ACT, Inc. national testing centers. Quieter test centers, greater flexibility in scheduling, you select the location, date, and time live on the Internet. Several Arizona Locations. You are required to use your own books and calculators for ACT Testing. Arizona assessments are still processed through this site. However, the format is slightly different than in the past. You must present a state issued photo ID such as drivers license to take an assessment. Note: Each assessment is now video and audio recorded. Please make sure you register for the appropriate assessment for the respective jurisdiction. CSA develops and delivers assessments. The jurisdiction establishes which assessments are recognized and/or required. We have tried to compile some information to help participants with some jurisdictions. We can not guarantee the accuracy of this information as jurisdictions may change codes or local requirements. If there is an error, please email us so we can look into it. Phoenix requires every fire protection business installing alarms, sprinkler systems, underground fire mains, etc. to have a Business Certificate Holder qualified representative on file. This is in addition to other requirements such as insurance certificate and registrar of contractor license, etc. This person must take a level 4 exam such as UFM4, ASCR4, or FA4 assessment for Business Certificate Holder, or acceptable alternative. A level 4 assessment will also satisfy requirements for level 2 on-site exams for that person. Please verify with the jurisdiction what your company will need. Peoria and Phoenix require that a competent on-site person be present on a job site any time related work is being performed. A competent on-site person must also be present for any and all inspections. The individual must have a CSA photo ID card, or temporary documentation if an assessment was just passed. Please check with the jurisdiction to verify exact requirements. Code editions currently used on tests include: Check the "Watch Your Time" page in the Assessment Overview Manual to see which codes are currently tested on. NFPA documents can be purchased from the National Fire Protection Association at www.nfpa.org. PLEASE NOTE: If you do not put the company name on your CSA ID card, Phoenix will REQUIRE each person to obtain a separate ID card from them at an additional fee. This is to identify what companies insurance you are working under. Each company is required to have insurance on file as freelancing is not allowed. If you put your company name on the CSA card and it is for an On-Site qualification you will not be required to obtain an additional card or pay an additional fee.
